Insurance:

Therapists at Thrive Counseling accept most commercial insurances; however we do not accept Medicare or Medicaid at this time.  As therapy may be covered in full or in part by your insurance, Thrive asks that clients contact their insurance company to determine coverage for mental health services.  Therapists can provide a Superbill for you to submit to your insurance to obtain out-of network benefits if applicable.

When you provide us with your insurance information, we do our best to determine in advance what your benefits are and what the cost of services will be; however, information provided to us by insurance companies can be unreliable, and we are unable to guarantee accurate information until the first submitted insurance claim has been fully processed, which can sometimes take weeks.  If you are uncertain of your benefits, please contact your insurance company directly to inquire about the cost of outpatient mental health services.

If you decide not to use insurance, you will be required to sign an "opt-out form" which confirms this decision.  Signing this form means that you cannot request that these sessions later be billed to insurance and that these sessions will not count towards any deductible or out-of-pocket maximums.

Please note that all payments are due at the time of service.

***Please note that some insurance companies DO NOT cover couples therapy sessions, resulting in us only accepting self-pay rates for those services.  If you are uncertain if your insurance covers couples therapy, please directly contact your insurance company to obtain this information.

Self-Pay

Self-pay rates are set according to clinician's experience in the field, training, and cost of living.  These rates are available upon request but typically follow the following rate schedule:

​

Initial Intake Session:  $250

Therapy Session (55 min):  $200

Therapy Session (45 min): $150

Couples Session (50 min):  $250

Consultation (30 min): $50

Sliding Scale​

​To provide care to a diverse range of clients while maintaining quality and consistency of therapy, sliding scale services are offered in limited quantities by each therapist.  Needs adjustment will be re-evaluated every 3 months, with rates being adjusted accordingly.  To qualify for sliding scale, self-pay clients are invited to provide our office manager with the following information:​​

          * income level / place of employment

          * household responsibilities / bills

          * sense of financial stability / instability

This process relies on trust and accountability as sliding scale services impact both the accessibility of your therapy services and the sustainability of our practice.

***Please note that sliding scale is NOT available for insurance clients as insurance rates are mandated by legally contracted rates.

Payment Policy

In order to provide services, we require a valid payment card on file in our Electronic Health Records system.  Payment is due at time of service.

​

Late cancellation is defined as cancelling with less than 24 hours notice or joining a session more than 15 minutes past the start time of session.  Any late cancellations will incur a $100 fee.  

​

Please note that any balances on your file must be cleared at he beginning of the next session or earlier to avoid cancellation of services.

Fees for Additional Services:

Letters and Forms (ESA, surgery, FMLA, Workers Compensation): 

$50 for first page, $25 for each additional page

Records Requests:

$25 Records are prepared and mailed after receipt of payment and completed Release of Information.

Court Appearances:

Clients are discouraged from having their clinician(s) subpoenaed for court.  While your attorney may initiate the subpoena request, it does not necessarily mean that the clinician's testimony will be solely in your favor.  The clinician will only testify to the facts of the case and of their professional assessment.  Please be advised that your attorney is responsible for the court appearance and testimony fees.

Phone Calls:  $150 per hour

Emails:  $15 per email

Letters:  $50 per page

Testimony:  $150 per hour

Mileage:  $0.55 per mile

Time away from office due to court appearance:  $150 per hour
